  • The date your health insurance coverage will begin depends on the date you enroll. If you enroll during the first fifteen days of any given month, your coverage will begin on the first day of the following month. If you enroll during the second half of any given month, your coverage will begin on the first of the second following month.

  • Evidence of new disability.
